About Your Member ID Card

When you enroll with Group Health, you and each member of your family covered under your plan will receive a member identification card.

What's On Your Card

You can see what all the information on your card means by following the number guide below. It will help you when you're getting care, or talking to Customer Service or a consulting nurse.

  1. Your name
  2. Your plan member ID number
  3. Your plan name, also called provider network
  4. Numbers that help us process your pharmacy claims
  5. Additional provider networks you can use at either in- or out-of-network benefit levels

Reverse Side of All Cards

On the back of your ID card, you'll find helpful contact numbers for Customer Service and our Consulting Nurse Service, as well as an address when you want to submit a claim.

Member Cards and Networks

Members with Access PPO, Alliant Plus, Options, and Options PPO plans may access regional care in Oregon, Alaska, Montana, Idaho, and parts of Washington state through the First Choice Health provider network. Alliance, Alliant Select, Group Health, and Options Select members may access First Choice Health for urgent and emergency care at discounted rates outside of our service area.
First Choice Health

Members with Access PPO, Alliant Plus, Options, and Options PPO plans may access the First Health Network for care nationwide (outside of Washington, Oregon, Idaho, Alaska, and Montana). Alliance, Alliant Select, Group Health, and Options Select members may access First Health Network for urgent and emergency care at discounted rates outside of Washington, Oregon, Idaho, Alaska, and Montana.
First Health Network

Access PPO, Alliant Plus, Options, and Options PPO plans allow use of pharmacies in the MedImpact network at either in- or out-of-network benefit levels.

Order Replacement or Extra Cards

If you are logged in, you can use the reorder form to replace a lost card or to order an extra copy for a covered family member.  Or, you may contact Customer Service.
